NOTE TO AREA PARENTS OF HIGH SCHOOL ATHLETES:
Planning to attend this Saturday's free sports clinic? Please be advised that sports forms should be filled out BEFORE coming to the clinic at BCMH. Taking that important step will help speed up the process. Athletes will probably need their parents' help in answering some of the questions about family medical history and other details. Even though forms will be available at the hospital on the day of the clinic, we ask that athletes pick up a form from their coach or high school office, and bring it on Saturday completely filled out and signed, including a parent signature.
