Reminder: For those who would like to participate in athletics this upcoming school year, you must have a current physical on file in order to begin practicing, no exceptions.
Physicals are now current for two years, so for those who received one in 2019, it is now good until 2021. However, you must still complete the Annual Requirement pages of the physical form.
We will no longer take hard copies of physicals. Once you have completed a physical, you must go to the link below to upload a picture of your physical. There you will also complete the Annual Requirement pages. It will also ask for other health and insurance information.
